North Carolina law requires all students enrolled in public and charter schools to have specific immunizations and health documentation on file. Families have 30 calendar days from the first day of attendance to submit required records. |
Required Immunizations
Requirements for Kindergarteners - 6th Grade

Consent for Medical Care Form
Maureen Joy collects annual health information to help ensure the safety and well-being of every student.
The Consent for Medical Care Form is a required school document that allows parents and guardians to:
- Authorize the school nurse to provide basic medical care during the school day
- Share important information about their students’s medical history and ongoing health conditions
- Report allergies, chronic illnesses, and other health concerns
- Provide information regarding head injuries or concussion history
- Consent to comfort measures (such as ice, heat packs, eye wash, etc.)
This form is the primary way our school nurse becomes aware of a student’s medical needs. Without this information, we may be unaware of important health conditions that could impact your student’s safety during the school day.
In certain emergency situations, schools operate under N.C. Gen. Stat. § 90-21.1, which permits necessary medical care for minors when immediate action is required. However, completing the Consent for Medical Care form allows the school to act more efficiently and in accordance with your preferences.
All families should plan to complete this form fully and accurately at the beginning of each school year. Please submit one form per student. Click the button below to submit the form electronically. For first-time students, the form is embedded within the school’s enrollment application.
Out-of-State and Transfer Students
Students Entering a North Carolina Public School for the First Time
In accordance with North Carolina law, all students entering a North Carolina public or charter school for the first time — regardless of grade level — must provide:
- A completed North Carolina Health Assessment Transmittal Form, performed within 12 months prior to the student’s start date, and
- A valid immunization record demonstrating compliance with all vaccines required by North Carolina law.
Students transferring from another state must meet North Carolina immunization requirements, even if their previous state had different standards.
Transfer Students from Another North Carolina Public School
For students transferring from another public school within North Carolina, our school will request health and immunization records directly from the previous school. However, if records are not received or are incomplete, the responsibility to provide required documentation remains with the parent or guardian.
If we are unable to obtain the necessary health assessment or immunization records, families will be notified and must submit the required documentation within 30 calendar days of notification. Failure to provide required documentation within the 30-day period may result in exclusion from school in accordance with North Carolina law.

NC Health Assessment Transmittal Form
Pursuant to N.C. Gen. Stat. § 130A-440 and § 130A-441, certain students are required to submit a completed North Carolina Health Assessment Transmittal Form upon school entry.
The NC Health Assessment is a physical examination completed by a licensed medical provider. The exam must be performed within 12 months prior to the student’s first day of attendance.
The form must be the official North Carolina Health Assessment Transmittal Form.
Who Must Submit this Form?
- All Kindergarten students
- All students transferring from out of state
What Does NOT Count?
The following documents do NOT replace the NC Health Assessment form:
- Sports physical forms
- Well-child visit summaries
- After-visit summaries
- Generic physical forms from other states
When Is It Due?
Families have 30 calendar days from the student’s first day of attendance to submit the completed NC Health Assessment form.
Students who do not submit the required documentation within the 30-day period are subject to exclusion in accordance with North Carolina law.
Exemptions
North Carolina law permits limited exemptions from required immunizations under specific circumstances. These exemptions are governed by N.C. Gen. Stat. G.S. 130A-156 and 130A-157. In the event of a vaccine-preventable disease outbreak, students with exemptions may be excluded from school for their protection and the protection of others, as directed by public health authorities.
Medical Exemptions
Under N.C. Gen. Stat. § 130A-156, a student may be exempt from one or more required immunizations if a licensed medical provider certifies that a vaccine is medically contraindicated.
A valid medical exemption must:
- Be completed and signed by a licensed physician (MD or DO) or other authorized healthcare provider
- Specify the vaccine(s) for which exemption is requested
- State the medical reason for the exemption
- Indicate whether the exemption is permanent or temporary
The school must receive official documentation from the healthcare provider. Temporary medical exemptions must be updated if the condition changes.
Religious Exemption
North Carolina allows a religious exemption in accordance with N.C. Gen. Stat. § 130A-156 and 10A NCAC 41A .0403.
A religious exemption:
- Must be a written statement of bona fide religious belief
- Must be submitted by the parent or legal guardian
- Does not require a specific state-issued form
- Must clearly state that immunization conflicts with the family’s religious beliefs
Philosophical or personal belief exemptions are not permitted under North Carolina law.
